Access to the Plane of Disease
Walkthrough
This quest flags players for access to the Plane of Disease. Once completed, players may return to the Plane of Disease whenever needed, but should always bring a Zombie Flesh Suit when attempting to use the portal.
Before You Begin
Before starting the quest, obtain a Zombie Flesh Suit. This item can be crafted, but it is often easier to purchase one from auction.
It is also recommended to collect the three Deathknight hearts before logging the quest. The Deathknights are not triggered spawns, and gathering the hearts first saves a return trip after speaking with Necrologist Zizclank.

Step 1: Obtain Deathknight Endraval's Heart
Deathknight Endraval can be found in Takish Hiz (W).
From the Oasis coach, travel southwest toward Takish Hiz (W). Continue past the area on the western side until you reach the dead trees beyond it.
Deathknight Endraval spawns randomly among the dead trees. He sees through invisibility.
Defeat Deathknight Endraval and loot his heart.
Step 2: Obtain Deathknight Bileheart's Heart
Deathknight Bileheart can be found near the rift in Salisearaneen.
From Castle Lightwolf, travel straight south into Salisearaneen (N). Continue south until you reach a rift surrounded by dead trees. One end of the rift reaches Salisearaneen (NE), while the other reaches Salisearaneen (E).
Defeat either version of Deathknight Bileheart and loot his heart.
Step 3: Obtain Deathknight Halik's Heart
Deathknight Halik can be found near the border of Fort Alliance Center and Fort Alliance South.
From Brog Fens, travel straight south toward the border of Fort Alliance Center and Fort Alliance South. Once you reach the border area, travel west until you find a small circle of rune stones.
Defeat Deathknight Halik and loot his heart.
Step 4: Speak with Necrologist Zizclank
After collecting all three Deathknight hearts, travel to Dead Hills (NW).
Necrologist Zizclank and Necrologist Xeptip can be found near the derelict zombie camp north of the Dead Hills portal. The camp contains three mausoleums, with the center mausoleum leading down to the Dark Portal (PoD).
Speak with Necrologist Zizclank to log the quest. Turn in the three Deathknight hearts as required to gain access to the Dark Portal (PoD).
Step 5: Enter the Mausoleum
After completing the heart turn-in, enter the center mausoleum.
Fight through the derelict enemies outside and inside the mausoleum as needed, then continue down toward the Dark Portal (PoD).
Use the Dark Portal (PoD) to enter the sanitorium area.
Step 6: Speak with Fatima Ubel
Once inside the sanitorium, follow the hall until you reach Fatima Ubel.
Speak with Fatima Ubel. During this step, Last House Orderlies mobs may become aggressive and attack. Defeat any orderlies that spawn or aggro.
Fatima Ubel sends you deeper into the ward to retrieve what has been left in containment.
Step 7: Retrieve the Crystallized Residue
From Fatima Ubel's room, continue to the hallway leading toward the room with the bathtubs.
Approach the end of the hall near the tubs to receive a pop-up. This grants the Crystallized Residue and causes additional enemies, including Last House Orderlies and Last House Healer mobs, to spawn.
Defeat the spawned enemies as needed, then continue the quest.
Step 8: Speak with Rebo Chilins
After obtaining the Crystallized Residue, continue past Fatima Ubel and locate Rebo Chilins.
Speak with Rebo Chilins. Additional Last House Orderlies mobs may spawn during this step. Defeat them if they attack.
After speaking with Rebo Chilins, return to Fatima Ubel.
Step 9: Return to Fatima Ubel
Speak with Fatima Ubel again.
She sends you to the Last House Pharmacist to obtain the substance he guards.
Step 10: Defeat the Last House Pharmacist
Find the Last House Pharmacist in the first room near the entrance area.
Important: The questor must be the same player who speaks with the Last House Pharmacist and loots the quest item. If one player triggers the encounter and another player loots the item, the quest may not advance properly and may need to be deleted and restarted.
Speak with the Last House Pharmacist to trigger the encounter. The pharmacist, along with additional enemies such as a Last House Healer and Last House Orderlies, will become hostile.
Defeat the Last House Pharmacist and loot the required item, Pharmacist's Wasp Venom.
Return to Fatima Ubel once the item has been looted.
Step 11: Speak with Medelonia Virel
After returning to Fatima Ubel, she sends you to Medelonia Virel.
Travel farther down the hallway past Rebo Chilins to find Medelonia Virel. Speak with her to continue the quest. Additional orderlies may spawn during this step.
Medelonia Virel explains that you must appear as something the ward recognizes and sends you to obtain a Last House Physician Robe.
Step 12: Defeat the Last House Physician
Continue down the hallway to the final room containing the Last House Physician.
Speak with the Last House Physician to trigger the encounter. The [Last House Physician]], a Last House Healer, and an Last House Orderlies will become hostile.
Defeat the Last House Physician and loot the Last House Physician Robe.
Important: The player who triggers the Last House Physician encounter should loot the Last House Physician Robe.
Step 13: Speak with the Orderlies
After obtaining the Last House Physician Robe, proceed to the small hallway near the physician's room and speak with the Last House Orderlies at the end.
The Last House Orderlies will recognize you as a physician and allow you to pass to the other side of the wall.
Once you have passed through this point, the Last House Physician Robe is no longer needed for future access to this section.
Step 14: Travel Through the Second Floor
After passing through the Orderlies wall, you will reach the second floor section of the Last House.
Last House Surgeon spawns on the second floor of The Last House. When aggroed, it will spawn three Zombie (PoD) mobs.
If you only want to pass through the first room, use invisibility and hug the wall while moving past the two Scourge Rat mobs.
Scourge Rat, on the second floor, do not see through invisibility, but Last House Orderlies do.
Move carefully and avoid getting too close to the surgeons unless your group is ready to fight.
Step 15: Obtain or Craft a Zombie Flesh Suit
To gain access out of the second floor and use the final portal, you need a Zombie Flesh Suit in your inventory.
A Zombie Flesh Suit can be crafted by a tailor with at least crafting level 10.
Crafting materials:
Stitched Zombie Skin drops from Zombie (PoD), in the Last House.
Sinew Cord drops from the Master Surgeon.
First, combine three Stitched Zombie Skin items to make a Zombie Swatch. Do this twice to create two Zombie Swatch items.
Then combine the two Zombie Swatch items with the Sinew Cord to craft the Zombie Flesh Suit.
Important: The Zombie Flesh Suit must be in your inventory to receive the pop-up at the Oozing Portal and leave the Last House.
Step 16: Reach the Final Portal
After passing through the second floor area, proceed carefully through the next section.
Go up the ramp, then continue down the hall to the left. Turn left again and follow the hallway until you reach the final portal.
Some enemies in this area may see through invisibility, while others may not. Move carefully and avoid unnecessary pulls when possible.
Step 17: Enter the Plane of Disease
At the final portal, Oozing Portal, make sure the Zombie Flesh Suit is in your inventory.
Once you pass through the portal, you are flagged for the Plane of Disease.
Notes
- Dung Flies are non-targetable NPCs inside The Last House Chambers. They can aggro if players use area-of-effect abilities.
- Piles of bones and remnants found throughout The Last House Chambers will apply a damage-over-time effect to nearby players.
- Always bring your Zombie Flesh Suit when using the portal to the Plane of Disease.
- The Last House Physician Robe is only needed to gain access through the Orderlies during the quest.
- Large groups may take time to move through the final portal because the port is on a timer.
- Deathknight hearts can be collected before logging the quest.
- The questor should personally trigger and loot required items from the Last House Pharmacist and Last House Physician encounters.
